Severe Mental Illness (SMI) Support Pathway
Dedicated mental health support services for individuals living with severe mental illness.
About This Pathway
“Our Severe Mental Illness Pathway is a commitment to better understanding, compassionate intervention, and seamless support at every stage of the journey. By strengthening early detection and ensuring smooth transitions into NHS care with clear, trusted signposting, we are breaking down barriers that too often delay help.
This pathway is about more than services; it is about restoring hope, dignity, and connection. Through coordinated, community-focused care, we are creating a future where mental health is recognised early, supported consistently, and nurtured within a community that empowers every individual to thrive.”
